NOTICE TO PATIENTS

Please carefully review the information contained in this notice.

In order to allow you to make a fully informed decision about your health care, the physicians of Nephrology Associates, P.A. (the “Practice”) would advise you that at some point during the course of your treatment, you may be referred to one of the following outpatient Fresenius Kidney Care (FKC) dialysis centers in which certain physicians of the Practice own an indirect financial interest:

(i) FKC Central Delaware; (ii) FKC Mid Sussex; (iii) FKC Milford; (iv) FKC Greentree; (v) FKC Rehoboth; (vi) FKC North Dover Dialysis Center; (vii) FKC Smyrna; (viii) FKC Millsboro; (ix) FKC Dover Home Dialysis Center: (x) FKC Bridgeville; (xi) FKC Elkton; (xii) FKC Hockessin Dialysis; (xiii) FKC Middletown.

Please note that you are free to select the outpatient dialysis centers listed above or outpatient dialysis centers other than those set forth above for your treatment.  For more information about alternative dialysis providers, please ask your physician or a member of our staff.  We will provide you with names and addresses of other providers that are near to your home or place of work.
